The forms listed below have been customised for use by schools and colleges for which Hampshire County Council is the Local Authority monitoring the NQT induction process,  schools should use these versions.

At the beginning of each NQT’s induction year, schools & colleges should download a complete pack of 2 forms, to be retained by the Induction Tutor for eventual completion. To download the forms, click on the links below and print. There are separate packs of forms for schools and 6th form colleges. If you wish to save them on your hard drive for future use, click on ‘file’ then ‘save as..’ and save in the appropriate folder.

Please note that the previous stand alone "Interim Assessment Form has now been merged into the 1st and 2nd Assessment form.  Also there is no longer two seperate Final Forms (one for pass and one for fail) they have been merged into one Final form.

Every section, including all of the first page, must be completed, and the forms must be signed and dated. No part of the form, other than the section for NQTs’ comments, should be completed by the NQT. The forms should be returned to the NQT Co-ordinator at the address shown on the final page, to arrive one week before the end of terms 1 and 2 and two weeks before the end of term 3.
