End of Life care
Hampshire County Council Adult Services and Hampshire Primary Care Trust are working in partnership with independent and voluntary sector providers to provide a broad range of services for people at end of life, across Hampshire. We hope this site provides information and support for someone who is dying and for their carers, to enable them to achieve the best possible quality of life especially during the final stages of their illness.

Help the Hospices have a search facility to help you find a hospice or palliative care service outside of Hampshire.
Tel: 02075 208222 - Hospice at Home - run by the Rosemary Trust offering palliative care for people in their own homes.
